Students to participate in health survey

PUSD students will be asked to be a part of the California Department of Education's Healthy Kids Survey (HKS).    This survey will be administered to all 5^th, 7^th, 8^th, and 11^th graders during in the fall of 2011.   This is a very important survey that will help us promote better health and safety among our youth, combat problems such as drug abuse and violence, and gather information to identify needs for grant proposals.   The survey asks questions about health-related behaviors, experiences, and attitudes.  It also asks questions about use of alcohol, tobacco, and other drugs; bullying and school safety; and environmental and individual strengths. 

Participation is voluntary, confidential and anonymous.  If you do not want your child to participate, you must notify the school and complete a Withdrawal Form at least a week before the survey is administered.  You can examine the survey at the front office or at the PUSD office.
